This survey, Anabolic/Ergogenic Use by Adult Athletes, is being conducted by Dr. James
Langenbucher and his colleagues at Rutgers University. The goal of the study is to
characterize the demographics, medical history, physical training history, motivations for
physical training, goals and identity as a physical trainer, prohormone use, anabolic use,
over-the-counter and illicit ergogenic/thermogenic use, and future plans for performance-
enhancing drug use, by adult recreational and competitive athletes. The survey will
canvass each of these areas briefly, and will take about 20 minutes to complete.
Your participation in this survey is completely VOLUNTARY. YOU MAY DECIDE
NOT TO PARTICIPATE AT ANY TIME DURING THE SURVEY. If you would like to

In order to participate in this study you must be at least 18 years of age. You need not
have use anabolic steroids, ergogenics, or any other drug in order to participate. We
will NOT ask for any information which would positively identify you (e.g., name, social
security number, etc.), and we will NOT attempt to identify you. Internet Protocol (IP)
addresses will automatically be collected with each submission. IP addresses are strings
of digits that can be used to identify the computer from which the submission originated,
but not the individual who made the submission. IP addresses will be recorded merely to
ensure that there is only one response from each address--we want to make sure that
each person answers the survey only once, and to do that we need to keep a temporary
log of IP addresses--but they will be purged at the end of the study.
Every effort will be made to ensure the confidentiality of your responses. However,
confidentially cannot be guaranteed under all foreseeable circumstances, and it is
conceivable that confidentially could be compromised. No one will contact you further
regarding the study unless you specifically request that by providing contact information
along with a written request for contact to the researcher. The results of this study will
only be reported collectively, rather than on an individual basis.
The questions of this survey ask you specifically about the use of Schedual III drugs and
other illegal/illicit drug use. It is possible that some of these questions may be upsetting
to you. If that occurs the researchers will not provide counseling, but participants will be
referred to the Principal Investigator, Dr. James Langenbucher, a licensed psychologist,
who will make every reasonable effort to encourage that individual to seek appropriate
care. You may choose not to answer any questions that disturb you. IF you have any

This survey is taken at no financial cost, and there will be no monetary payment for
participating. The survey takes approximately fifteen to twenty minutes to complete. By
participating in this study you will provide some benefit to society by furthering scientific
understanding of the relationship between elite physical training practices such as
weightlifting and bodybuilding, and the use of anabolic steroids, ergogenics, and other
agents. Please answer the questions honestly and take the survey only once.
