This survey, "The Effects of Bodybuilding and Weightlifting Training
Pracitices on Mood in Adult Athletes", is being
conducted by Pilar M. Sanjuan, M.S., Dr. James Langenbucher, and
colleagues at Rutgers University. The goal of the study is to examine
moods associated with bodybuilding and weightlifting practices. The
survey will canvass these areas fairly briefly and will take about 20
minutes to complete. We plan to recruit approximately 500 respondents.
Your participation in this survey is completely VOLUNTARY.

In order to participate in this study you must be at least
18 years of age.
There are no other limitations on your participation. We will NOT ask
for any information which would positively identify you (e.g., name,
social security number, etc.), and we will NOT attempt to identify you.
Internet Protocol (IP) addresses will automatically be collected with
each submission, in order to exclude duplicate submissions (the same
person taking the survey twice) from the data. IP addresses are strings
of digits that can be used to identify the computer from which the
submission originated, but not the individual who made the submission.
IP addresses will be recorded merely to ensure that there is only one
response from each address -- we want to make sure that each person
answers the survey only once, and to do that we need to keep a temporary
log of IP addresses - but they will not be linked to your survey responses
and they will be destroyed at the end of the study.
This research is confidential. Confidential means that the
research records will include some information about you, such as your
birthday and your IP address. I will keep this information confidential
by limiting individual's access to the research data and keeping it in
a secure location. The research team and the Institutional Review Board
at Rutgers University are the only parties that will be allowed to see
the data, except as may be required by law. If a report of this study
is published, or the results are presented at a professional
conference, only group results will be stated, unless you have agreed
otherwise. Every effort will be made to insure the confidentiality of
your responses. However, confidentiality cannot be guaranteed under all
foreseeable circumstances, and it is highly unlikely, but conceivable,
that confidentiality could somehow be compromised. No one will contact
you further regarding the study unless you specifically request such
contact by writing Pilar M. Sanjuan at The Center of Alcohol Studies,
Rutgers University, 607 Allison Rd., Piscataway, NJ, 08854-8001,
providing contact information along with a written request for contact
to the researcher.
The questions of this survey will ask you specifically about
the use of Schedule III drugs and other illegal/illicit drug use. It is
possible that some of these questions may be upsetting to you. If that
occurs the researchers will not provide counseling, but you may phone
Dr. James Langenbucher, a licensed psychologist, at 732-445-0908, who
will make every reasonable effort to encourage you to seek appropriate
care. You may choose not to answer any questions that disturb you. If
you have any questions about your participation in the study please

This survey is taken at no financial cost, and there will be no
monetary payment for participating. The survey takes approximately
twenty minutes to complete. By participating in this study you will
provide a benefit to society by furthering scientific understanding of
the relationship between mood and elite physical training practices
such as are used in weightlifting and bodybuilding. Please try to answer
all the questions, answer the questions honestly, and take the survey
only once.
